Arthroscopic Subacromial Decompression (No Rotator Cuff Repair)

Updated Evidence-Based Insights (2023–2024)

  1. ASD Alone Offers No Structural Repair
    • Recovery is pain-limited and function-focused — no tissue healing timelines.
    • Full AROM expected within 4–6 weeks in uncomplicated cases.
  2. No Need for Prolonged Immobilisation
    • Sling for comfort only, usually <1 week.
    • Early motion prevents stiffness and deconditioning.
  3. Scapular Control & Neuromuscular Training is Key
    • Emphasise scapular setting, CKC drills, proprioception from week 1.
  4. Avoid High-Load Early Overhead Work
    • Return to heavy or repetitive overhead work should be based on neuromuscular control, not time.
  5. Consider Associated Procedures
    • If biceps tenotomy/tenodesis or ACJ excision done, progressions will vary.

Post Surgery rehabilitation guidelines

Phase Timeline Goals & Interventions Avoid
Phase 1: Early Mobility & Control 0–2 weeks – Discontinue sling as tolerated
– AROM + AAROM
– Table/wall slides
– Scapular setting
– Isometric RC
– CKC (e.g., weight shifts on wall)
– Heavy lifting
– Sustained overhead activity
Phase 2: Recruitment & Functional ROM 2–4 weeks – Full active ROM expected
– Proprioception drills
– Light scapular retraction + RC control
– Introduce therabands
– Light closed chain loading
– Poor form/compensations
– End-range overload
Phase 3: Strengthening Phase 4–6 weeks – Functional strength (deltoid, cuff, scapular)
– Dynamic scapular drills
– Begin endurance (light loads, high reps)
– CKC and open chain mix
– Overhead resistance if control poor
Phase 4: Return to Activity/Work 6–12 weeks – Restore strength and control through range
– Gradual return to lifting
– Full functional patterns (push/pull)
– Rapid return to heavy overhead work
Phase 5: Performance Integration 12+ weeks – Overhead power, sports, and endurance
– Only progress if neuromuscular control is consistent
– May include high-level plyometrics or loaded overhead drills
– Repetitive overhead fatigue without tolerance

Progression Criteria

  • Pain-free full ROM
  • Good scapular control during elevation
  • Symmetrical strength and proprioception
  • Endurance with light loads before power

Testimonials

“Kate is a superb therapist and has moved me quickly from despair to recovery.”

Alex

”Kate has terrific skills and sensitivity – she treats the whole person, not just the symptoms”

Emma

”FIVE STAR EXPERIENCE.  Kate looks at the body as a whole (both physically and emotionally) and is able to identify and treat the root cause of the pain.  This type of Physiotherapy is much more logical and effective than traditional Physiotherapy.  The best physio I have come across – thank you Kate.”

Stacey

“I cannot sing enough praises for the treatment I received from Kate. Instead of treating the symptoms, she treated the cause of the problem.”

Tara (a professional triathlete)

“Kate used a technique called visceral release to work on my internal organs, the effect has been amazing. I have had several sessions with Kate and the pain in my hip has almost gone. I can move freely and walk more easily.”

Mary

“I have been seeing Kate on and off for several years now. She is incredible, no matter what my ailment and I have had various over the years, she has always managed to reduce my pain and increase my mobility. She is very professional but keeps a great balance of being personal too. I have been to many other physios but I will not see anyone else now.”
